Record one incident of a target behaviour: what led up to it, what it looked like, and what happened afterwards. Fill it in as soon as you can, while the detail is still fresh.

Describe what you saw, not what you think it meant.

  • Activity — the setting: what was going on and what the participant was doing.
  • Antecedent — the trigger: the one thing that happened immediately before.
  • Behaviour — what it actually looked like, in plain observable terms.
  • Consequence — what happened straight afterwards, including how you responded.

What was happening in the lead-up: where, with whom, and what was going on.

The single thing immediately before it — a request, a noise, a change of plan.

What someone else watching would have seen and heard. How long did it last?

How you responded, and what the participant got or avoided as a result.

Your report goes straight to the coordinator for review.
