ENGAGEMENT ROUTE0/2

A support worker who is rostered, directed and integrated into Keana Care will commonly be engaged as an employee. Do not use a contractor agreement only because the worker has an ABN.

How is this worker engaged? (required)
Duties actually assigned to this worker (required)

Tick only what this worker will really do. The guide asks for the child-related, driving and task-specific evidence below only where the duty is assigned — the rest can be marked "Not applicable".

Record the date as each one is finished.

Candidate Application and Interview Assessment completed. (required)
Two independent Reference Check Records completed and assessed. (required)

Two, and independent of each other — not two people from the same referee.

Employee / Contractor Classification Assessment supports the chosen arrangement. (required)

An ABN on its own is not a classification. The assessment has to support the route chosen above.

Support Worker Position Description completed and signed. (required)
Employment Agreement completed with employment type, SCHADS coverage / classification, pay, hours and start date; then signed. (required)
Employee Personal and Payroll Details Form completed and stored in the restricted payroll folder. (required)

Restricted folder — payroll details are not filed with the rest of the worker file.

Official Fair Work Information Statement provided. (required)
Official ATO TFN Declaration and super choice process completed; payroll / STP setup completed. (required)

Verify the evidence yourself rather than taking the worker’s word for it, and file what you sighted.

Photo identification verified. (required)
Australian work-right evidence verified, where required. (required)
National Police Certificate obtained and assessed. (required)
NDIS Worker Screening clearance verified / linked. (required)

Keana Care's normal standard is to require it. Where the director decides otherwise, that decision is written down — record where it is filed.

WA Working with Children Check verified before child-related work, unless a lawful exemption applies. (required)

Where an exemption applies, record which one and where the decision is filed.

First Aid and CPR are current for assigned duties. (required)
Driver licence, vehicle registration, roadworthiness and insurance verified before driving / transport duties. (required)

Only where driving or participant transport is part of the role.

Manual-task, medication and other task-specific training / competence verified only for duties actually assigned. (required)

Verified before the task is performed, and after participant-specific competency verification.

Certificates and expiry dates recorded in the Worker File and Credential Checklist. (required)

List what expires and when — police certificate, screening, First Aid / CPR, licence — so the next check is not missed.

Optional. Stored privately with this record — never in a shared folder.

JPG, PNG, HEIC, WEBP or PDF, up to 5 MB each. You can drop files here.

Every line here must read "Done" before the worker’s first shift.

Worker Code of Conduct signed. (required)
Confidentiality, Conflict and Fitness Declaration signed. (required)
Worker Induction Checklist completed with the facilitator. (required)
NDIS Worker Orientation Module evidence saved. (required)
Worker has read incident, complaint, privacy, safeguarding, risk, emergency and record-keeping procedures. (required)

Not "has been sent" — has read.

Participant-specific orientation completed for the assigned participant. (required)
Worker-participant match, competence, availability and any restrictions approved. (required)
Director / manager approves the worker for the first shift. (required)

Signed on the review link below, once everything else on this page is complete.

What is recommended, what is conditional on the duties assigned, and what a director needs.

RECOMMENDED — NDIS Worker Screening. Not generally mandatory for workers of an unregistered provider, but recommended by the NDIS Commission and adopted as Keana Care’s normal safety standard. Issued by the NDIS Commission / WA.

CONDITIONAL — Working with Children Check. Required for child-related work unless an exemption applies. Issued by the WA issuer.

CONDITIONAL — Driver / vehicle evidence. Required only where driving or participant transport is part of the role. Obtained from the worker and their insurer.

CONDITIONAL — Medication / manual-task training. Required before performing the relevant task and after participant-specific competency verification. Keep the training and competency evidence on file.

DIRECTOR — Direct-support documents. A director who personally delivers support completes the same worker file. A non-supporting director does not need every support-worker certificate.

Is this worker a director who personally delivers support? (required)

Use the Employment Agreement for an employee. Use the Independent Contractor Agreement only if the real working arrangement is genuinely independent and the classification assessment supports it. Keep the contractor documents in the OPTIONAL ONLY folder.

Which agreement was signed? (required)

Official checks — these links were checked on 13 August 2026: NDIS worker screening for unregistered providers (ndiscommission.gov.au/workforce/worker-screening/worker-screening-unregistered-providers), the NDIS Code of Conduct (ndiscommission.gov.au/rules-and-standards/ndis-code-conduct) and the Fair Work information statements (fairwork.gov.au/employment-conditions/information-statements).

Optional — anything about this engagement the next person should know.
